They agreed to provide services (project, work, or job), but did not provide the services as promised.

Examples:

• The contracting company promised to come and remove trees, but they never showed up and you had to choose a pricier alternative.
• You paid the contractor in advance, but they never completed the work.
• You hired a professional (e.g. accountant, house cleaners, etc) to help with a job, but they did it poorly and won't refund the money.
• You canceled a service within the timeframe, but they refuse to refund you the money.
